Registration

Delegates must submit a registration form and wear their name badges at all times. No casual attendance will be permitted.

Please complete the registration form to register for the course.

Selection process
Applicants that registered will undergo a selection process and all successful candidates will be notified of their acceptance after 20 March 2013. Payments will be required after notification of selection.

Registration fees: R7 000 excluding VAT
Please note that the registration fee does not include accommodation or airport transfers or tours.

Payment of fees?
Payments can be made by personal credit card or bank/wire transfer. Please clearly indicate your name and surname on the deposit slip. Receipts will only be issued to delegates after receiving proof of payment. A copy of the deposit slip must be faxed to Deon Salomo at +27 21 938 0238 or email to deon.salomo@mrc.ac.za.

Credit Card
Only Master and VISA credit cards will be accepted

  • A photocopy of the front and back of the credit card;
  • Expiry date;
  • The last three digits on the reverse side of the card; and
  • The card holder's identity document or passport must be faxed together with the Registration Form

An authorisation letter should also accompany the registration form authorising the Medical Research Council to deduct all the necessary payments

All information provided will be handled in the strictest confidence

The organisers will not be responsible for identifying funds if the delegate's name and initials are not mentioned.

Cancellations and refunds
Cancellations must be sent in writing to Mr Deon Salomo, South African Medical Research Council, PO Box 19070, Tygerberg 7505, Cape Town, South Africa or by fax on +27 21 938 0238. For cancellation before 15 May 2013 a cancellation charge amounting to 25% of the registration fee will be deducted from the refund. No refund for cancellation will be granted for any reason after 15 May 2013. The payment for refunds will be processed after the conference.
